Maritime has over 4700 collection sites as of October 2012. We assign
members a collection site as close to your business location as
possible. However, at times you may need to find an additional or
alternate collection site for you or one of your crewmembers. You may
use the links below to find a new site.
Please
note: The collection sites listed on the following web pages will only
see individuals with the required drug testing paperwork provided
by Maritime. Individuals who walk in without the required paperwork
will not be seen.
Individuals may obtain the required drug
screening paperwork from The Maritime Consortium, Inc. Please call
1.800.775.6985, Monday – Friday, 9AM to 5PM EST.
Maritime uses two Lab Networks with separate Chain of Custody forms for
each. If you wish to go to an alternate collection site, it must be
one of the sites that matches your paperwork. In order to determine
the correct link to use below, please refer to the Federal Chain of
Custody Form in your test paperwork. This is the 5-part carbon copy
form. If you have LabCorp paperwork, it will say LabCorp at the top
left hand corner of the form. If you have Quest paperwork, it will say
Quest Laboratories in the upper right hand corner of your paperwork.

If using the link above, you will need to provide a zip code and a
range to search around the zip code for a site. Click on the
"Occupational urine drug screen collections" radio button and then on
"SEARCH" once you've entered your zip code and range.

If you are going to use the Quest Diagnostics site locator link, you
will need to enter a zip code and select "Drug Screen - Urine" from the
"Reason for Testing" pull down menu and then "FIND".
NOTE: Quest has two kinds of sites designated either as PSC or
Preferred. The designation is part of the site name, e.g. Bestgate
PSC, or Farmington (Preferred). If you are currently assigned to a
Quest Diagnostics PSC, you may not go to a Preferred site. You may
always go to a PSC site if you have any Quest Diagnostics paperwork.
