USCG Drug Testing Form for Non-Members

 If you wish to take a USCG Drug test for your license, license renewal, or upgrade without joining the Maritime Consortium random drug testing program, please fill this form out.

A USCG drug test is required for all Coast Guard captain’s licenses, except modifications, duplicates, and International Endorsements.

Licenses that require a drug test include the OUPV/Six-Pack and 25-Ton, 50-Ton, and 100-Ton captain’s licenses.

Once you have taken your drug test at the collection site, it will be 2-3 business days before Maritime receives the result. We will email the result to you and also put it in the mail.
